What is DPC?

Direct Primary Care is a membership based healthcare model for primary care.

What does that mean? For an affordable monthly fee, you become a member of my primary care practice, which means any care that I can provide for you myself is covered by your membership. Extra costs such as lab and radiology costs will be transparent so you will know exactly what they will cost beforehand. You should still carry some type of insurance because membership does not include prescriptions, emergency room visits, hospitalizations, or visits to specialists.

Imagine living near a family member who just happens to be a family physician. You will have my office and personal cell phone number. Just as you wouldn’t call your uncle at work repeatedly, there has to be some boundaries, but you should have the comfort of having direct access to a family doctor committed to meeting your healthcare needs. The business plan for Uyemura Family Medicine, PLLC. is to provide personal medical care to a membership family of 600 people. What would it be worth to you to be a member of that family?

Ages 0-17 — $15 sign up fee, then $15/month*
Ages 18-39 — $50 sign up fee, then $50/month
Ages 40-64 — $60 sign up fee, then $60/month
Ages 65+ — $80 sign up fee, then $80/month

Unfortunately, if you have Colorado Medicaid it is illegal for me to sign you up and bill even a deeply discounted membership fee. However, if children of a household have Colorado Medicaid, but the parents do not, it will still be possible to enroll your family.
